1. Simply go to the website of DLS (Direct Laboratory Services): www.directlabs.com.

2. Order the #1 Comprehensive Wellness Profile, pre-paid for $89. The #1 CWP includes the specific Chem Profile and CBC necessary for their Evaluation plus a T4, T3, and TSH.

3. Select a convenient, near-by DLS affiliated blood draw site when ordering their #1 CPW.

4. Take the requisition to your near-by draw-site and have your blood drawn.
NOTE: Please have your blood drawn approximately four hours after your regular breakfast or lunch, NOT after an overnight fast.

5. When you receive your test results, contact Health Equations at 800-328-2818 to arrange for your Evaluation and Consultation.

• Many physicians and other healthcare professionals use the Health Equations BTE in their practices. If your health provider is one of them, you may obtain your Blood Test Evaluation and Consultation from him or her.
