How To Get Started
Your HEq Evaluation is generated from lab test results that you provide to Health Equations.
Obtaining Required Lab Tests for the HEq Evaluation
It is important to schedule your blood draw for the Required Lab Tests 3-4 hours after a usual breakfast or lunch. The Evaluation is designed to determine cell and tissue function/dysfunction during the day. Although blood samples drawn after an overnight fast can be used for the Evaluation, they are less informative.

Not available in MA, MD, ND, NE, NJ, NY, RI, SD or VT. Skip to OPTION #2.
Order the Comprehensive Wellness Profile (CWP) at WWW.DIRECTLABS.COM or call 1-800-908-0000. The CWP includes a chemistry panel, lipid panel, complete blood count (CBC) and thyroid function tests. Current cost is $97.
You will receive a blood draw requisition from Direct Labs. Then locate the nearest LAB CORP draw site to you at www.labcorp.com. Authorize Direct Labs to send your blood test results to [email protected] by completing the Sign Release HIPAA form on “Your Orders” page.
Direct Labs does not offer service in 9 states – MA, MD, ND, NE, NJ, NY, RI, SD and VT. If you live in one of these states see Option #2.
Option #2
Order the Chemistry Panel and Complete Blood Count (CBC) at Life Extension, www.lef.org/Vitamins-Supplements/ItemLC381822/Chemistry-Panel-Complete-Blood-Count-CBC-Blood-Test.html. Non-member cost is $47.
Note: This Life Extension (LE) Panel includes the same tests as the CWP minus the thyroid function tests. Thyroid function tests are not required for the HEq Evaluation. You can however order thyroid function tests from LE, www.lef.org/Vitamins-Supplements/ItemLC304131/Thyroid-Panel-TSH-T4-Free-T4-Free-T3-Blood-Test.html. The non-member cost is $100.
You will receive a blood draw requisition from Life Extension. Then locate the nearest LAB CORP draw site to you at www.labcorp.com. Once you have your test results forward them to HEq, Contact Information.
Obtaining Other Lab Tests
If you wish to order other lab tests for review in Consultation or use otherwise, you will find lists of tests offered at LE, www.lef.org/Vitamins-Supplements/Blood-Tests/Blood-Tests.htm and at Direct Labs, www.directlabs.com/OrderTests/tabid/55/language/en-US/Default.aspx.

Instructions for Consultations
Complete the Intake Forms and return to Health Equations along with any other labs or reports you would like to review with Lynne. Be sure to include a list of supplements and medications you are currently taking.
For Repeat Evaluation Consultations or Follow-up Consultations simply complete and return the Health Survey Form, pages 3 and 4 of the Intake Forms .
Evaluation and Consultation Fees
First Evaluation: $310 including 45-minute Consultation with Lynne August
Repeat Evaluations: $210 including 30-minute Consultation with Lynne August
Follow up Consultations: $90 for 30-minute Consultation with Lynne August
Medical Record Review: $30 -$90 for review of medical records submitted before and discussed during a Consultation.
This fee is dependent on the number and complexity of records.
Payment and Scheduling Information
Once your labs, Intake Forms and additional reports have been submitted to HEq, please contact [email protected] or call (800) 328-2818 to make your payment and schedule your Consultation appointment. Your Evaluation will be performed and sent to you after your payment is received.
